REPORTING SYSTEM
The University of North Texas Health Science Center (UNT Health) evaluation team developed reporting systems for SVPP programs and training & technical assistance (T/TA) providers to streamline site data collection. These reporting systems satisfy state and federal reporting requirements. The reporting systems are designed to allow implementers and providers to input data on a monthly or quarterly basis, as preferred. Although SVPP programs and T/TA providers have different reporting systems, the due dates, technical assistance instructions, and data correction procedures remain the same for both groups.
FY2026 DUE DATES
The SVPP reporting system must be submitted on a quarterly basis, by the dates listed below.
- May 30, 2026 - Quarter 1 (February, March, and April)
- August 30, 2026 - Quarter 2 (May, June, and July)
- November 30, 2026 - Quarter 3 (August, September, and October)
- February 28, 2027 - Quarter 4 (November 2026, December 2026, and January 2027)
**DATA CORRECTIONS REMINDER: If you need to submit data corrections for a previously submitted report then please scroll down to the section titled "Submitting Data Corrections" below. Fill out the form in its entirety and reference question numbers for any of the items you need to change. You will receive a copy of the form via email when you press "submit".**
